A leading provider of senior care is currently recruiting for a Nurse Practitioner. We are located off the 5 freeway for easy on/off access in downtown San Diego.

We are currently looking for an LVN with Clinical Experience. The LVN for this position will work under the direction and supervision of the RN Clinic Supervisor. This position requires the following: Using and practicing basic assessment (data collection) Participating in planning, execution, and interventions in accordance with the care plan. Contributing to the evaluation of individualized interventions related to the care plan. Administer medications, implement primary care orders, and evaluate participant complaints physically and/or by telephone. Provide education to participant, and caregiver in regards to care plan, nursing and treatments or medications. Communicates participant’s condition to primary care givers, physician and multidisciplinary team. Skills / Requirements
Broad knowledge base of health care problems of the frail elderly.
Knowledge of medical equipment and instruments. Knowledge of common safety hazards and precautions to establish a safe work environment. Skilled in identifying problems and recommending solutions. Skilled in preparing and maintaining records and writing reports, and responding to correspondence. Skilled in developing and maintaining department quality assurance. Skilled in establishing and maintaining effective working relationships with participants, medical staff, staff members and family caregivers. Ability to react calmly and effectively in emergency situations. Knowledge of medications used in geriatrics and adverse reactions. Skilled in wound care. Knowledge of home care services nursing principles and practices, with particular reference to the elderly. Important Notes
A minimum of one year’s experience in working with the frail or elderly population required. IV Certification preferred. Shall have either training or related experience in the job assigned.
