Job: Nurse Practitioner - Part Time

Under the supervision of the Center Director the Nurse Practitioner will provide primary care to participants. You will perform physical assessments of new PACE Enrollees, semi-annual reassessments of participants and develop and implement appropriate plans of care to Community eldercare of San Diego (CESD) PACE program participants. You will evaluates participant physical complaints and provide appropriate treatment. You will provides participants and caregivers teaching and education. We ask the you demonstrate the knowledge and skills necessary to assess, plan, care for, and provide services to frail elder participants according to assigned responsibilities and CESD standards.

Education
Graduate of a school of professional nursing required.
Licensed as an RN in the State of California.
Certified as a Nurse Practitioner by a national certifying organization recognized by the State of California Board of Nursing.
Certified as a Geriatric Nurse Practitioner with prescriptive authority preferred.

Experience
A minimum of one year’s experience in working with the frail and elderly population required. Two years experience as a nurse practitioner in a geriatric setting required. Shall have either training or related experience in the job assigned.
