The REACH Case Manager is a full-time, exempt position reporting to Southern Caregiver Resource Center's (SCRC) Associate Director. The REACH Case Manager position works from one of the South Bay workstations provided by the Program's partner agencies and provides Spanish and English REACH "Early Intervention" psychoeducational programs as well as case management, care planning and consultation, and support services to caregivers and families of individuals with Alzheimer's disease or related dementias. The REACH Case Manager also co-facilitates a support group and works closely with the Program’s Promotoras on "Prevention" outreach activities.
Essential duties and responsibilities: * Leads REACH Early Intervention psychoeducational programs in Spanish and English. * Performs comprehensive, formal needs assessments with family caregivers both at their homes and in the office. * Develops care plans and provides ongoing consultation with clients. * Provides clients with information on and assistance in accessing relevant community resources. * Facilitates caregiver support group(s). * Provides education and support services to family caregivers. * If licensed or in the process of licensure, provides short-term counseling to caregivers. * Organizes and participates in educational programs as needed. * Collects data on all facets of help provided to families using formats compatible with SCRC’s monthly, quarterly and annual reports to its funding sources. * Performs outreach and marketing functions, including public speaking, to inform the community about services available through SCRC, particularly the REACHing Out program. * Attends community networking meetings and fosters productive relationships with a variety of individuals and organizations. * Fosters and maintains cooperative and productive working relationships with all REACHing Out collaborative partners. Ensures ongoing excellence, consistency and seamlessness in the delivery of services to clients through the collaborative partners. * Fosters and maintains cooperative working relationships with all SCRC staff. * Complies with all SCRC services standards and protocols. * Carries out required REACHing Out grant activities and reports such activities to the Associate Director. Maintains and manages other projects and performs other duties as assigned. Skills / Requirements
* Masters degree in a clinical and/or health/aging discipline (MSW, MFT, MA, MSG).
* Understanding of the Hispanic/Latino culture, health disparities and issues related to access of services. * Exceptional written and oral communication skills in English and Spanish. * Excellent public speaking skills in English and Spanish and demonstrated experience. * Knowledge and skills to thoroughly assess the needs of family caregivers including knowledge related to a variety of chronic and disabling health conditions, relevant community resources, respite care, helpful interventions, long-term care planning options, and issues related to caregiver mental health. * Familiarity with health and mental health agencies, social service agencies and formal/informal support networks. * Ability to maintain thorough and accurate case records and documentation. * Organizational skills, accuracy and attention to detail. * Computer literacy and ability to use a variety of software programs including Microsoft Word and Excel. * Excellent interpersonal skills and the ability to interact effectively and professionally with a variety of staff, clients and collaborative partners. * Possession of a valid driver license and vehicle for use on the job. * Ability to work a flexible schedule as needed. Important Notes
$40-46K annually to start, DOE. Position works 40 hours per week, typically Monday through Friday.
