Job: Social Worker - ADHC

We are looking for an experience Social Worker for our new Adult Day Health Center to provide social work services for the Center. Process all inquiries in a timely manner and maintains tracking documentation. Meets with family members, or referring agency, and determines appropriate program. Provides in-home assessments for all ADHC participants.

High School Diploma or GED. Bachelor’s in Social Work preferred. CA Driver License. Two years experience in community based organization or social services. CPR certification. Must be Bilingual in English/Spanish.
