Job: Registered Nurse (RN) Care Coordinator - FT for Escondido

Registered Nurse (RN) Care Coordinator, Full Time, to coordinate integration of primary care medical services with behavioral health at a north county mental health agency site. The Nurse Care Coordinator will be employee of a north county community health center and part of a care team. Duties to include working with mental health staff to screen adult clients at risk for diabetes, obesity, hypertension, and cardiovascular disease. Organize clinics and work with the Nurse Practitioner in screening, assessing, and treating persons with serious mental illness. Coordinate follow up care. Responsible for data entry and tracking. Participate in designing and implementing wellness programs and client engagement strategies for improved health status. California RN license is required.
