Job: Ambulatory Care Coordinator

Jobing Description
The Ambulatory Care Coordinator will be responsible for the maintenance of the Coumadin Clinic; reviewing labs, calling patients, adjusting Coumadin Dose based on algorithm, documenting contact in Coumacare and NextGen, and will support the development and maintenance of additional ambulatory care services.

ESSENTIAL FUNCTIONS:

The above is not intended to be an all-inclusive list of essential functions for the job described, but rather a general description of some of the responsibilities necessary to carry out the duties of this position.

Coumadin Clinic

* Enrolls patients into Centre Coumadin Clinic, adds patient information to Coumacare database and NextGen Coag. Database (overlap until lab PAQ).
* Reviews daily anticoagulation lab results and notifies patients of routine lab results within the same business day, but no later than 24 hours from sampling time
* Notifies SNF of results same day, to include faxing or phoning orders for dosing and F/U testing
* Following CFHC anticoagulation protocol, makes necessary adjustments in warfarin (Coumadin) dosing on a daily basis
* Demonstrate self injection techniques for patients requiring LMWH

SNAP (sleep Study) Liaison

* Instruct patients in the use of home sleep study devices, maintain dispense log, receive reports and alert provider to results.

Additional Care Management / Quality Improvement Duties

* Identification and outreach to patients missing chronic or preventive care
* Post-discharge follow-up calls
* Assist in the monitoring of quality and affordability data by physician
* Recurring renal function data query for commercial (SH) patients submitted to business office
* ABI (just started) for assessing PVD in commercial (SH) patients
* Other duties as assigned

Skills / Requirements
Skills: Excellent verbal communication and problem solving skills; strong computer skills (EMR, Word processing and spreadsheet software, data registries)

Knowledge: Principles of disease management, including anti-coagulation therapy; principles of outpatient case management; principles of quality improvement

Abilities: Establish and maintain effective working relationships with those contacted in the course of work, patients, physicians, and the medical office staff; effectively provide patient education/demonstrations; assess and evaluate patient understanding of education provided


REQUIREMENTS: Requires 2 years experience and a current LVN license. Strong clinical background preferred. Knowledge of California’s Pay for Performance model desirable.

While performing the duties of this job, the employee is frequently required to stand, walk, twist, bend, reach, grasp, and pull charts. The employee must occasionally sit, squat/kneel, lift up to 25 lbs, and use wrist deviation, hand/wrist repetitions and manual dexterity. Specific vision and hearing required by the job includes close eye work, and normal hearing.



WORK ENVIRONMENT: Centre for Health Care maintains a drug free workplace, and smoke free working environment. Employees are required to successfully pass a pre-employment drug screen. Smoking is permitted in designated areas outside of Centre offices.

* This position may involve exposure to a variety of infectious and communicable diseases.
* This position works in a well-lit, air-conditioned, busy clinical setting with moderate noise level. This position is frequently exposed to hazardous exposures, and may occasionally need personal protective equipment when in contact with patients.
