Jobing Description
•Provide case management services to include interviewing, assessing, case planning, solution seeking, goal setting, referral and advocacy, life skills training, and family building
•Remain culturally sensitive and respectful in all client and staff interactions
•Identify individual/family strengths that can be applied to overcoming identified family challenges that prevent or hinder the family form becoming self-sufficient
•Help families identify goals and specific methods of achieving set goals
•Assist, guide, and motivate families toward self-sufficiency as defined by agency standards and funding sources
•Provide client advocacy and life skills training as needed by individual clients
•Provide support in the form of reflective listening, emotional support, strengths identification, challenge identification and solution seeking as needed
•Maintain ongoing contact with FSS participants to build trusting relationships, provide support, serve as a referral source, and monitor progress toward identified goals
•Coordinate Emergency Assistance to individual clients, which may include provision of basic needs such as food, clothing, shelter, crisis intervention, information and/or referral
•Make home visits as part of the client assessment and family development processes
•Maintain detailed case files and document data and statistics that are required by program and funding sources

Skills / Requirements
•BA in SW or related field, Master’s preferred
•3 years of job-related experience including case management and direct service experience with at-risk, multiple problem families and children in both clinical and home based settings
•Experience with and respect for diverse cultures
•Experience with youth, family and community development methods
•Strong skills in interviewing, rapport building, assessment, goal setting, and motivating individuals and families
•Ability to work independently as well as cooperatives with other agency and program staff
•Experience working on collaborative projects
•Bilingual a plus
